Menno Place Advance Care Planning (ACP) education sessions CANCELLED due to precautionary measures with COVID-19.

Advance Care Planning

Advance Care Planning (ACP) encourages you to think about and share your values, wishes, and beliefs about your future health care.

Putting a plan into place and expressing your wishes:

  • Gives you more control over the care you receive now and in the future
  • Eases the burden on the people who support you should they have to make decisions for you
  • Will help you feel less anxious and more at peace

Do you have a plan?

Talk about your wishes with your substitute decision maker, loved ones, and health care team.

  • Record

Record your wishes in a way that works for you. Write them down or consider making a video.
